We look forward to meeting you at your first appointment.  Please take a moment to print and complete the Medical History which applies, as well as the Child Sleep Questionnaire or the “Snoring is No Joke” and Pain Questionnaire for adults.  All of these forms are required for patient registration and will be discussed at your New Patient Exam appointment.

If you have a dental insurance plan please ensure you have checked the Financial Info Tab for the required forms you will need to bring with you to have your claims submitted electronically.
